Bank Authorization Form

For ACH debits to my checking account, I understand that because these are electronic transactions, these funds may be withdrawn from my account as soon as the above-noted transaction date. I acknowledge that the origination of ACH transactions to my account must comply with the provisions of U.S. law. I certify that I am an authorized user of this bank account and will not dispute Lieder Digital, LLC billing my account so long as the transaction corresponds to the terms indicated in this authorization form.

In the case of an ACH transaction being rejected for Non-Sufficient Funds (NSF) I understand that Lieder Digital, LLC, may at its discretion attempt to process the charge again within 1-2 days, and I agree to pay any bank service fees related to the NSF.

Further, I agree not to hold Lieder Digital, LLC responsible for any delay or loss of funds due to incorrect or incomplete information supplied by me or by my financial institution.

This agreement will remain in effect until the payment schedule above expires, or Lieder Digital, LLC receives written notification of cancellation from me to cancellations@liederdigital.com. The notice of cancellation must be received in such time and in such manner as to allow sufficient time for processing.
